Laboratory Testing
Information Request Form

If you have any questions about doing a test, or interpreting the results, contact us by filling out the form below and clicking on the "Submit" button.

Although, the laboratory provides the largest single source of objective, scientific data on patient status, it is only one part of a complex biological picture of health or disease. As professional clinical laboratory scientists, our goal is to assist you in understanding the meaning of your laboratory results. It is important that you communicate with your physician so that together you can integrate the pertinent information, such as age, ethnicity, health history, signs and symptoms, laboratory and other procedures (radiology, endoscopy, etc.), to determine your health status.

Please provide all the test results/parameters obtained for a given panel (CBC, Chem-12, etc.). The pattern of results is often more important than a single abnormal result in evaluating laboratory data to identify underlying disease or to monitor disease processes. It would be helpful if you would include some information about yourself (age, sex, signs and symptoms, etc.) so we can focus our responses to better answer your questions.
