1. ASCLS Members $75.00 per Participant

3. Special CLS Student Rate $25.00 per Student with valid I.D. Students must print out the text-only registration form and mail or fax it, along with proof of student status, to the address below.

5. Hard Copies of the Basic QC Practices Manual by Dr. James O. Westgard may be purchased by on-line course participants for an additional charge of $30.00 (a $60 value) on the Basic QC Practices online registration form.

ASCLS will accept Visa, MasterCard and American Express and you may register online through our secure internet server (online registration allows immediate access to the course materials and test). Credit Card orders may be also be faxed to 301-657-2909 (manual processing delays access to materials and test). Checks and MOs should be made payable to ASCLS and mailed with your registration form to:
